DS-11 Application Form
This form is for applicants who:

Cannot present their last passport (lost or stolen);

Have never been issued a US passport;

Hold a passport issued more than fifteen years ago;

Hold a five year passport.

If one of the above apply to you, please come personally to the Embassy with one or more of the following documents:
- U.S. birth certificate or U.S. Consular Report of Birth Abroad;
- One of your previous U.S. Passports;
- Certificate of Naturalization;
- Current Photo ID.
You will also need to bring:

Photographs:
- Two identical 2" x 2" (5 cm x 5 cm) color photographs taken within the past 6 months; full face, frontal view against a plain white or off-white background; size of head should be between 1 inch and 1 3/8 inches (2.5 cm and 3.5 cm) from the bottom of the chin to the top of the head. Over 16 years old:
US$ 100.00 (Dkr. 550.00) .

Under 16 years old:
US$ 85.00 (Dkr. 467.50).
- Payment is accepted by cash, credit card (when presented at Consular Section), or Danish kroner check payable to "U.S. Embassy".

The Passport and Citizenship Unit is open to the public between 9:00 and 12:00 noon daily, except Wednesdays, American and Danish holidays.
