Appointment Booking Check List

BEFORE YOU BOOK AN APPOINTMENT PLEASE MAKE SURE THAT YOU HAVE ALL THE NECESSARY DOCUMENTS TO BRING TO YOUR INTERVIEW.

All applicants must present following documents at the interview:

YOUR VALID PASSPORT

If your passport is damaged (spine is broken), or you do not have a page for a visa, please get a new passport before your interview.

ONE PHOTOGRAPH (Attach to your DS-156 Form)

One color or black and white photograph, less than 6 months old, against a light colored background, full frontal view, i.e., looking directly at the camera. >> (more...)

ORIGINAL JYSKE BANK RECEIPT FOR APPLICATION FEE

The application fee of Danish kroner 700.00 must be paid at any Jyske Bank branch in Denmark into Reg. No. 5013, account number 200200-2. Payment by Internet banking is not acceptable. Please note that no refunds will be made for any fees that are transferred incorrectly. The application fee is non-refundable. The original fee receipt must be presented when the visa application is submitted at the Embassy. Only applicants for A-1, A-2, A-3, G-1, G-2, G-3, G-4, G-5, C-3, NATO, and U.S. Government-sponsored visas are exempt from the application fee.

COMPLETED DS-156 APPLICATION FORM

When you click the "BOOK YOUR APPOINTMENT" link you will be directed to the visa application form DS-156. This form must completed onlinein order to schedule your appointment for a visa interview.

COMPLETED DS-157 Supplemental NIV Application for:

- all men between ages 16 and 45 regardless of nationality;
- all visa applicants (men and women) from Cuba, Iran, Libya, North Korea, Sudan and Syria, who are 16 and over;
Applicants for A, G and NATO visas are not required to complete form DS-157. Consular officers may, at their discretion, request form DS-157 from applicants not listed above on a case-by-case basis.

ENVELOPE

A medium-sized, self-addressed stamped envelope with DKr. 76.00 in postage for your passport to be returned by registered mail. You will not receive your visa at the interview. We will mail your passport when we have finished processing your visa. The Embassy offers NO PICK-UP SERVICE .

Besides these 6 essential items, certain visa categories require additional documentation. If you fall into one of the following categories, do you have the additional documentation? (Note: this is not an exhaustive list and presentation of these documents does not guarantee visa issuance.)


Are You a Tourist or Business Traveler? (B Visa)

PROOF OF FINANCE: Proof that you can support yourself during your entire stay in the U.S. ;

EMPLOYMENT LETTER: Proof of employment, study, or other commitments that will demonstrate that your stay in the U.S. will be temporary;

LETTER FROM YOUR EMPLOYER: Description of your business plans while in the U.S. and anticipated period of stay. You may be required to provide a job contract or pay statements.


Are You a Student or an Exchange Visitor? (J, F, or M Visa)

COMPLETED DS-158 APPLICATION FORM.

PROOF OF FINANCE: Proof that you can support yourself during your entire stay in the U.S. ;

COMPLETED DS-2019 APPLICATION FORM. If you are on a J exchange program, you must have a completed Form DS-2019 that matches exactly the name in your passport;

COMPLETED I-20 A-B APPLICATION FORM. Form I-20A-B (for F1 students) or Form I-20M-N (for M-1 students) properly completed and signed by both the Designated School Official (DSO) and the applicant;

SEVIS FEE RECEIPT. Most J, F, and M applicants must now pay a SEVIS fee. Please refer to Student Visas or Exchange Visitor Visas for details and instructions on payment. Please bring an electronic receipt or I-901 receipt to your interview.

Marriage and Birth Certificates If your Spouse and/or children will be traveling with you, please bring a marriage certificate and birth certificates for your dependents

Are You a Temporary Worker (H, L Visa) or Athlete or Artist (O, P Visa)?

I-797 NOTICE OF ACTION: Original I-797, Notice of Action, and copy of the petition your employer filed with U.S. Citizenship and Immigration Services (USCIS).

BLANKET L-1 PETITION FEE (BLANKET L VISAS ONLY): The U.S. $500 fee must be paid to the Consular Cashier at the Embassy after the interview by cash or credit card. This fee should not be paid into the Embassy's Jyske Bank account. If you have an individual petition approval notice (I-797), you need not pay this fee.

Marriage and Birth Certificates If your Spouse and/or children will be traveling with you, please bring a marriage certificate and birth certificates for your dependents.

Are you a Treaty Trader Applicant (E Visa)?

DS-156E:  E-visa applicants need to submit an original DS-156E.  Download the DS-156E.

Are You Working on a Ship or for an Airline? (C1/D Visa)

LETTER FROM EMPLOYER: A letter from your employer confirming your employment and your position.

Are You a Member of the Media? (I Visa)

LETTER FROM EMPLOYER: A letter from your employer stating occupation; whether your position is permanent or temporary and your purpose of travel and length of stay;

PRESS ID: Your credentials issued by a professional journalistic association;

CONTRACT OF EMPLOYMENT: Freelance media workers must also submit a valid contract of employment.

Remember:
-- Each member of a family requires a separate appointment.
-- If you do not have a valid e-mail address you can create a dummy address: initials @usvisa.dk (example: John Q. Doe = jqd@usvisa.dk)

RESIDENTS OF SWEDEN MUST APPLY THROUGH EMBASSY STOCKHOLM

Delivery of Visas: Most visa applicants will receive their visas within five working days of their interviews. However, some applications may require additional clearances and longer processing time. Please do not make firm travel plans until after you have received your visa.
